EVALUATE THE EFFECT OF MICROVARICOCELECTOMY ON SPERM DNA FRAGMENTATION INDEX IN INFERTILE MEN

The objective was to evaluate the effect of microvaricocelectomy on sperm DNA fragmentation index (DFI) in infertile men. Method: A controlled observational study was based on 205 patients who sought medical consultation at Hanoi Medical University Hospital. In which, 142 men were diagnosed with infertility who had unilateral varicocele belonged to the varicocele group and 63 men with normal reproductive health belonged to the control group. Results: The mean sperm DFI was significant higher in the varicocele group compared with that in the control group (31.8 ± 18.8 compared with 22.6 ± 10.6, respectively, p = 0.0002). In the varicocele group, sperm DFI was considerably reduced after surgery (31.8 ± 18.8% pre-surgery to 22.6 ± 10.6 post-surgery with p = 0.0001). The improvement was evidently observed in patients with severely damaged sperm DFI (≥ 30%). The mean post-operative sperm DFI was comparable with that in the control group (25.1 ± 14.9 vs 22.6 ± 10.6, p = 0.13). There was no remarkable differences in the percentage of severe sperm DFI (≥ 30%) between the varicocele group after surgery and the control group (26.7% vs 20.6% respectively, p > 0.05). A negative correlation between the change in post-operative sperm DFI and the level of pre-operative sperm DFI was also observed in this study (r = - 0.77, p = 0.0001 ). Conclusion: Microsurgical varicocelectomy reduced significantly the sperm DFI, therefore, this surgery should be indicated early for infertile men with varicocele
