Whole exome sequencing (WES) identified a homozygous c.234dup variant in the NKX6-2 gene relating to Spastic ataxia 8 with hypomyelinating leukodystrophy - the underlying cause of severe neurodevelopmental impairment in both families. The study contributed additional data on NKX6-2 variants in Vietnamese patients with intellectual disabilities and explored the genotype-phenotype correlation. Furthermore, it emphasized the importance of early genetic testing in children presenting with developmental delay, spastic quadriplegia, and unexplained nystagmus, especially when brain MRI findings suggested hypomyelinating leukodystrophy. This approach will provide a critical basis for diagnosis, patient management, and genetic counseling.
2025-05-30T00:00:00+00:00 Copyright (c) 2025 Tạp chí Nghiên cứu Y học https://tapchinghiencuuyhoc.vn/index.php/tcncyh/article/view/3499 Efficacy and safety of R-GEMOX in relapsed or refractory diffuse large B-cell lymphoma: A single-center experience in Vietnam 2025-04-22T13:11:39+00:00 Dinh Thi Hai Duyen Dinhhaiduyen1986@gmail.com Tran Thi Huyen tranhuyen1295hmu@gmail.com Nguyen Thi Mai Lan bsmailan.ubhn@gmail.com
Diffuse large B-cell lymphoma (DLBCL) is the most common histologic subtype of non-Hodgkin lymphoma (NHL), yet a substantial proportion of patients experience relapse or refractory disease following initial therapy. Among various salvage regimens, R-GEMOX - comprising rituximab, gemcitabine, and oxaliplatin - has shown promising efficacy and a tolerable safety profile. This retrospective cohort study aimed to evaluate the treatment outcomes and adverse events associated with R-GEMOX in patients with relapsed or refractory CD20-positive DLBCL treated at Hanoi Oncology Hospital. A total of 39 patients who received at least four cycles of R-GEMOX between January 2018 and May 2024 were included. Treatment response was assessed using the Lugano 2014 criteria, and toxicities were graded per CTCAE v4.0. The median age of the cohort was 57.2 years old. The overall response rate (ORR) was 69.2%, including a complete response rate of 28.2% and a partial response rate of 41.0%. Stable disease and progressive disease were observed in 7.7% and 23.1% of patients, respectively. Grade 3-4 hematologic toxicities occurred in 30.8% of cases. Median progression-free survival (PFS) was 9.5 months, while median overall survival (OS) reached 15.7 months. These findings suggest that R-GEMOX is an effective and tolerable salvage regimen in the management of relapsed/refractory DLBCL, particularly for patients ineligible for high-dose chemotherapy or stem cell transplantation.
2025-05-30T00:00:00+00:00 Copyright (c) 2025 Tạp chí Nghiên cứu Y học https://tapchinghiencuuyhoc.vn/index.php/tcncyh/article/view/3526 Evaluating the elimination of iCasp9-IL15 CAR-T cells using photosensitizers AP20187 in vitro 2025-04-11T16:08:52+00:00 Can Van Mao canvanmao@vmmu.edu.vn Le Duy Cuong leduycuong76@gmail.com
This study was conducted to evaluate the ability to eliminate iCasp9-IL15 Chimeric Antigen Receptor-T (CAR-T) cells using the photosensitizers AP20187 in vitro. Peripheral blood mononuclear cells (PBMC) were activated with Dynabeads Human T-Activator CD3/CD28 and IL-2 to optimize the concentration of the antibiotic blasticidin for screening iCasp9-IL15 CAR-T cells. iCasp9-IL15 CAR-T cells after proliferation with 1D2 artificial antigen-presenting cells, were screened using blasticidin. Subsequently, the iCasp9 suicide gene of these cells was activated at various concentrations of AP20187 photosensitizer. The results showed that after 5 days of culture, a blasticidin concentration of 15.0 µg/mL was used to screen iCasp9-IL15 CAR-T cells (activated, non-transformed PBMC showed > 95% cell death). The proportion of iCasp9-IL15 cells after blasticidin screening reached approximately 75%. Using AP20187 at the concentration of 20 nM resulted in almost complete elimination of iCasp9-IL15 CAR-T cells (survival rate: 2.14%). So, iCasp9-IL15 CAR-T cells can almost be eliminated by AP20187 at a concentration of 20 nM in case of high toxicity during treatment.
2025-05-30T00:00:00+00:00 Copyright (c) 2025 Tạp chí Nghiên cứu Y học https://tapchinghiencuuyhoc.vn/index.php/tcncyh/article/view/3535 Identification of variant of insulin receptor gene in resistant diabetes 2025-04-21T09:36:11+00:00 Nguyen Ngoc Khanh khanhnn@nch.gov.vn Can Thi Bich Ngoc ngocctb@nch.gov.vn
Insulin resistance is defined as a reduced biological response of target tissues to normal insulin levels and is a major mechanism leading to type 2 diabetes, particularly in obese individuals. Beyond obesity, other causes include medications (e.g., glucocorticoids, antiretrovirals, oral contraceptives), stress, pregnancy, dyslipidemia, insulin receptor autoantibodies (Type B insulin resistance), and genetic defects. Among genetic causes, mutations in the insulin receptor (INSR) gene can lead to severe insulin resistance known as Type A insulin resistance. This rare inherited disorder belongs to a spectrum of monogenic insulin resistance syndromes, including Donohue and Rabson-Mendenhall syndromes. We report a 12-year-old boy with diabetes due to an INSR mutation. He presented with polyuria, polydipsia, and weight loss. His height was 142cm (0 SD), and BMI decreased from 23.3 to 21.3 kg/m² (> 95th percentile). No acanthosis nigricans was observed. Lab results showed fasting glucose 16.8 mmol/L, HbA1c 7.8%, and HOMA-IR 11.2. Genetic testing revealed a novel heterozygous variant c.4115G>A (p.Arg1372Gln) of INSR gene; the mother carried the same variant, while the father was wild type. This study aimed to evaluate the initial treatment outcomes of LAGC patients receiving neoadjuvant FLOT at Hanoi Oncology Hospital. A descriptive cohort study was conducted on 35 patients with LAGC (cT3-4 and/or cN2-3, M0) treated with neoadjuvant FLOT between October 2021 and February 2024. Clinical data, radiological and histopathological responses, surgical outcomes, and adverse events were retrospectively and prospectively collected and analyzed. The median age was 62 years, with a male-to-female ratio of 4.8:1. The majority presented with epigastric pain (80%) and tumors located in the antrum (68.6%). Among all patients, 91.4% were cT4 (62.8% cT4b), and 97.1% had lymph node metastasis on imaging. Thirty patients (85.7%) completed 4 FLOT cycles. Radiologically, 50% had partial response (PR), 43.3% had stable disease (SD), and 6.7% showed progression. Of the 32 patients who underwent surgery, 29 (90.6%) had radical resection with an R0 rate of 100%. Pathologic complete response (pCR) was observed in 6.7% of cases. The postoperative lymph node-negative rate (ypN0) was 25%. Neoadjuvant chemotherapy using the FLOT regimen demonstrated favorable disease control, enabling high R0 resection rates in patients with locally advanced resect able gastric cancer. It is a leading cause of acute liver failure in many countries. This study investigated the acute toxicity and evaluated hepatoprotective effects on paracetamol-induced acute hepatitis of PROTECFUL hard capsules in Swiss mice. The acute toxicity was assessed through oral administration, and a fifty percent lethal dose was determined using the Litchfield-Wilcoxon method. In the paracetamol-induced acute hepatitis model, mice were orally administered distilled water, silymarin at 140 mg/kg per day, or PROTECFUL hard capsules at 345.6 and 1036.8 mg/kg/day. All indexes, including biochemical indicators (AST, ALT), liver malondialdehyde, glutathione concentration, and microscopic liver images, were evaluated. Regarding acute toxicity, our findings showed that the maximum tolerated dose was 35.4 times higher than the expected human dose without symptoms of acute toxicity. PROTECFUL-treated groups at both doses tended to decrease liver weight and lower liver enzyme activity compared to the model group. The histopathological examination of the liver in the PROTECFUL-treated groups tended to reduce liver degeneration and inflammatory cell infiltration. In summary, these findings suggest that PROTECFUL shows no acute toxicity and tends to have a hepatoprotective effect against paracetamol-induced liver injury.
2025-05-30T00:00:00+00:00 Copyright (c) 2025 Tạp chí Nghiên cứu Y học https://tapchinghiencuuyhoc.vn/index.php/tcncyh/article/view/3236 In vitro xanthine oxidase inhibitory and in vivo hypouricemic activity of dual herbal combination comprising Carica papaya L. and Piper lolot C. A well-motivated healthcare workforce can lead to improved patient satisfaction, reduced errors, and a more sustainable health system. This study aimed to describe the work motivation of healthcare workers in Bac Giang province in 2024 and analyze associated factors. A cross-sectional descriptive study was conducted among 253 health workers in Bac Giang province, Viet Yen and Yen Dzung districts using a quantitative approach with structured questionnaire. The main findings revealed that 80.6% of respondents were categorized as having low work motivation, with an average score of 3.66 (SD = 0.37). Significant associations were observed between work motivation and workplace location, years of working in the health field, and night shift frequency. Particularly, workers at the commune level exhibited the highest motivation (55.1%), compared to the district (24.5%) and provincial levels (20.4%). Similarly, workers with more than 10 years of working in the health field were more likely to have high motivation (91.8%) than those with 10 years or less (8.2%). Night shift frequency also highly associated to motivation, with workers taking more than eight shifts per month showing significantly higher odds of motivation (OR = 4.76, 95% CI: 1.22 - 18.65, p = 0.025). This syndrome represents the second most common genetic form of congenital hyperinsulinism in infancy. Children with HI/HA syndrome typically experience hypoglycemic symptoms triggered by fasting or high-protein meals and persistently elevated ammonia levels. We report a case of HI/HA syndrome in a 7-month-old female who presented with cyanosis and hyperammonemia, along with an initial normal glucose level at the time of presentation. About one week after the child’s admission to the hospital, we discovered that hypoglycemia was the cause of her irritability. The hypoglycemic episode was found to occur coincidentally with hyperammonemia. The combination of clinical findings, biochemical markers, and genetic sequencing identifying a GLUD1 pathogenic variant facilitated the correct diagnosis of HI/HA syndrome. As demonstrated by this case, the diagnosis of HI/HA syndrome requires a thorough clinical evaluation, comprehensive biochemical analysis, and genetic testing. Additional findings included persistent fontanelle, hyperopia with amblyopia, cortical thickening of long bones, and asymptomatic hypocalcemia with normal parathyroid hormone (PTH) levels. Brain MRI showed a thin pituitary gland. Case 2 presented with short stature (-4.96 SDS), a history of hypocalcemic seizures, and congenital astigmatism. She had normal facial appearance but showed cortical bone thinning, absent pituitary lobes, and hypocalcemia with inappropriately low PTH levels. Despite calcium supplementation, normocalcemia was achieved only after calcitriol therapy. Growth hormone was later initiated with favorable growth response.
