Kết quả điều trị hội chứng rối loạn sinh tủy bằng decitabine tại Viện Huyết học - Truyền máu Trung ương

Hội chứng rối loạn sinh tủy là một nhóm bệnh lý ác tính của tủy xương. Việc chẩn đoán và điều trị bệnh tương đối phức tạp. Những năm gần đây, các thử nghiệm lâm sàng sử dụng các thuốc giảm methyl hóa đã cho kết quả đầy hứa hẹn, mà điển hình trong đó là decitabine. Mục tiêu của nghiên cứu là đánh giá kết quả điều trị bằng phác đồ decitabine đơn trị trên bệnh nhân hội chứng rối loạn sinh tuỷ ở Viện Huyết học - Truyền máu Trung ương. Đối tượng nghiên cứu là 43 bệnh nhân được chẩn đoán hội chứng rối loạn sinh tủy theo WHO 2016 tại viện Huyết học - Truyền máu Trung ương từ 2018 đến 5/2020. Các bệnh nhân được phân nhóm nguy cơ theo tiêu chuẩn IPSS - R và được điều trị bằng phác đồ decitabin đơn trị. Nghiên cứu đã thu được một số kết quả như sau: tỷ lệ đáp ứng toàn bộ là 60,5%, trong đó 41,9% lui bệnh hoàn toàn và 18,6% lui bệnh một phần. Thời gian sống toàn bộ của bệnh nhân là 25,6 ± 2,9 tháng. Thời gian chuyển cấp trung bình ở 9 bệnh nhân là 21,8 tháng. Các tác dụng không mong muốn ở máu ngoại vi gồm giảm bạch cầu trung tính, giảm tiểu cầu và giảm hồng cầu với tỷ lệ lần lượt là: 27,9%, 18,6% và 41,9%. Có 9 tác dụng phụ khác thường gặp với tỷ lệ ở nhóm bệnh nhân nghiên cứu như sau: chán ăn 46,5%, đau đầu 37,2%, táo bón 32,6%, nôn 27,9%, ho 23,3%, sốt 18,6%, tăng men gan 18,6%, tiêu chảy 16,3% và viêm phổi kẽ 16,3%. Kết quả từ nghiên cứu này cho thấy phác đồ điều trị decitabine đơn trị đã cải thiện rõ tỷ lệ đáp ứng điều trị, kéo dài thời gian sống thêm với một số tác dụng không mong muốn thường gặp.
