Cắt lớp vi tính hai mức năng lượng trong đánh giá hẹp xơ vữa vôi hoá động mạch cảnh ngoài sọ

Sử dụng cắt lớp vi tính (CLVT) hai mức năng lượng (Dual energy Computed Tomographic: DECT) với kỹ thuật tạo ảnh xoá xơ vữa vôi hoá để đánh giá mức độ hẹp động mạch cảnh trong (Internal Carotide Artery: ICA) có so sánh với ảnh CLVT mạch máu thường qui (Computed Tomographic Angiography: CTA) trên 27 bệnh nhân với 43 động mạch cảnh trong bị hẹp. Mức độ hẹp được đánh giá theo NASCET trên cả hai kỹ thuật CTA thường qui và DECT có loại bỏ vôi hoá. Kết quả cho thấy có sự phù hợp với mức độ rất tốt giữa hai phương thức chẩn đoán này trong đánh giá mức độ hẹp ICA với hệ số Kappa = 0.812. Mức độ hẹp trung bình ICA đo trên hình ảnh CTA thông thường và DECT lần lượt là 63.3 ± 25.0% và 59.5 ± 24.4%, sự khác biệt có ý nghĩa thống kê (p < 0.001). Như vậy, DECT loại bỏ mảng xơ vữa vôi hoá đã tạo điều kiện thuận lợi cho việc đánh giá mức độ hẹp trong mọi trường hợp, như vậy nó phân loại hẹp ICA tốt hơn CTA thông thường.
