Viêm khớp thiếu niên tự phát thể hệ thống (Systemic Juvenile Idiopathic Arthritis-sJIA) là một bệnh tự viêm hiếm gặp ở trẻ em, với các triệu chứng sốt, viêm khớp và ít nhất một trong các biểu hiện sau: phát ban, nổi hạch toàn thân, gan to và/hoặc lách to, và viêm thanh mạc. Tuy nhiên, các triệu chứng lâm sàng ban đầu thường không đặc hiệu, dễ nhầm lẫn với các tình trạng bệnh lý khác. Tiêu chuẩn chẩn đoán sJIA của Hiệp hội Thấp khớp học Quốc tế năm 2001 đang được sử dụng hiện nay còn nhiều hạn chế trong chẩn đoán các ca bệnh không điển hình do đó gần đây, tiêu chuẩn Yamaguchi đã được áp dụng để chẩn đoán sớm sJIA. Chúng tôi báo cáo một trường hợp bệnh nhân 14 tuổi với biểu hiện sốt kéo dài kèm tổn thương ban da điển hình. Bệnh nhân được loại trừ các căn nguyên gây sốt khác và đủ tiêu chuẩn chẩn đoán sJIA theo tiêu chuẩn Yamaguchi. Kết luận: Ca bệnh minh hoạ việc áp dụng tiêu chuẩn Yamaguchi trong chẩn đoán sớm các trường hợp sJIA, giúp điều trị kịp thời và ngăn chặn các biến chứng nguy hiểm tính mạng.
