38. Hội chứng Fanconi và tình trạng thiếu máu nặng liên quan đến tenofovir ở bệnh nhân viêm gan B mạn tính: Báo cáo ca lâm sàng

Tenofovir disoproxil fumarate (TDF) được ghi nhận là một thuốc an toàn, được sử dụng trong điều trị HIV và viêm gan B. Tuy nhiên, thuốc cũng được ghi nhận có ảnh hưởng xấu lên chức năng thận trong một vài ca bệnh riêng lẻ. Chúng tôi báo cáo về một trường hợp hội chứng Fanconi ở bệnh nhân viêm gan B mạn tính, do tác dụng phụ đe dọa tính mạng người bệnh của TDF, sau 5 năm điều trị. Việc chẩn đoán được dựa vào kết quả xét nghiệm chức năng thận trước và sau điều trị can thiệp, kết quả mô bệnh học. Báo cáo của chúng tôi lưu ý các thầy thuốc về tác dụng phụ hiếm gặp, nghiêm trọng, để xử trí kịp thời khi sử dụng TDF trong điều trị viêm gan B mạn tính. Đồng thời, chúng tôi cũng thảo luận về các yếu tố nguy cơ gây độc thận liên quan đến TDF và đưa ra các khuyến nghị để theo dõi chức năng thận trong quá trình điều trị TDF.
