Chửa sau phúc mạc là thể bệnh hiếm gặp, chiếm < 1% tổng số chửa ngoài tử cung và thường khó chẩn đoán do không có triệu chứng đặc hiệu. Chúng tôi báo cáo ca bệnh: bệnh nhân nữ 34 tuổi, tiền sử cắt hai vòi tử cung, nhập viện sau chuyển phôi 40 ngày. Nồng độ β-hCG là 11,664 IU/L, nhưng không thấy túi thai trên siêu âm. Chẩn đoán nghi ngờ chửa trong ổ bụng nên chúng tôi chụp cắt lớp vi tính để tìm vị trí túi thai, sau đó phát hiện khối thai giữa động mạch chủ bụng và tĩnh mạch chủ dưới. Bệnh nhân được điều trị methotrexate đa liều trước khi phẫu thuật nội soi lấy khối chửa. Sau phẫu thuật, bệnh nhân ổn định và nồng độ β-hCG về âm tính sau 4 tuần. Tổng quan y văn cho thấy: chửa sau phúc mạc cần được nghĩ đến ở những trường hợp β-hCG tăng bất thường nhưng không xác định được vị trí thai. Phẫu thuật lấy khối chửa là phương pháp điều trị triệt để, mang lại tiên lượng tốt.
