Viêm khớp tự phát thiếu niên thể hệ thống (sJIA: Systemic Juvenile Idiopathic Arthritis) là một bệnh tự viêm với các biểu hiện hệ thống nổi trội (sốt kéo dài, phát ban, gan lách hạch to) xuất hiện sớm, kèm theo viêm khớp mạn tính sau đó. Bệnh thường bị chẩn đoán trễ do khó phân biệt với các nguyên nhân gây sốt kéo dài ở trẻ em, không có xét nghiệm đặc hiệu và hạn chế của các tiêu chuẩn chẩn đoán. Chúng tôi báo cáo một trường hợp trẻ nam 7 tuổi biểu hiện sốt cao kéo dài, phát ban trong cơn sốt và viêm khớp, xét nghiệm các chỉ điểm viêm tăng cao, được chẩn đoán ban đầu tại bệnh viện khác là mày đay cấp do nhiễm trùng (điều trị kháng sinh không đáp ứng) và sau đó là viêm khớp tự phát thiếu niên thể ít khớp (điều trị glucocorticoid và methotrexate 4 tháng nhưng không kiểm soát). Chúng tôi chẩn đoán trẻ mắc sJIA và kiểm soát bệnh hoàn toàn bằng thuốc tocilizumab. Chúng tôi cũng bàn luận về các tiêu chuẩn phân loại giúp chẩn đoán sớm sJIA và sự thay đổi trong cách tiếp cận điều trị bệnh.
