U tế bào mầm nội sọ biểu hiện 2 vị trí (bifocal) là nhóm bệnh hiếm gặp với khối u biểu hiện đồng thời ở vùng tuyến tùng và trên yên. Nghiên cứu hồi cứu trên 15 BN “bifocal” được điều trị hóa chất cảm ứng theo sau là xạ trị dựa trên phân tầng nguy cơ tại Bệnh viện K giai đoạn 2020 – 2024. Về mô bệnh học, 60% là germinoma và 40% là non-germinoma. Các BN đều là nam giới, tuổi trung bình 17 ± 5,8. Tại thời điểm chẩn đoán, 53,3% BN có đái tháo nhạt và 53,3% có tổn thương di căn. Về chỉ điểm u, có sự không đồng nhất giữa máu và dịch não tủy: 6/15 bệnh nhân (40%) có tăng β-HCG dịch não tủy > 50 UI/mL trong khi chỉ 2/6 trường hợp có tăng trong máu; 1/15 bệnh nhân có tăng AFP máu 123,1 ng/mL trong khi AFP dịch não tủy không tăng. Tỷ lệ đáp ứng toàn bộ tăng từ 40% sau hóa chất lên 60% sau xạ trị. Sau thời gian theo dõi trung bình 44 ± 18 tháng, 93,3% bệnh nhân chưa xuất hiện tái phát, di căn.
