Hồi phục tế bào T CD4+ sau ghép tế bào gốc đồng loài điều trị lơ xê mi cấp dòng tủy

Tế bào lympho T CD4+ đóng vai trò quan trọng trong điều hoà hệ thống miễn dịch và kiểm soát nhiễm trùng sau ghép tế bào gốc tạo máu đồng loài. Nghiên cứu hồi cứu được thực hiện trên 78 bệnh nhân lơ xê mi cấp dòng tủy được ghép tế bào gốc đồng loài từ máu ngoại vi tại Viện Huyết học - Truyền máu Trung ương, nhằm mô tả đặc điểm hồi phục tế bào T CD4+ và phân tích một số yếu tố ảnh hưởng trong 100 ngày đầu sau ghép. Kết quả nghiên cứu cho thấy tỷ lệ bệnh nhân hồi phục T CD4+ ≥ 200 tế bào/µL trong 100 ngày đầu là 65,4%. Xác suất hồi phục tích luỹ của T CD4+ tăng dần theo thời gian. Trong phân tích đa biến, kiểu ghép là yếu tố liên quan độc lập đối với hồi phục T CD4+ trong 100 ngày đầu, trong đó nhóm ghép nửa hoà hợp có khả năng hồi phục T CD4+ thấp hơn nhóm ghép hoà hợp hoàn toàn (HR = 0,28; p < 0,001).
