Mối liên quan của thang điểm CRISTEN và công thức máu với mức độ nặng trong hội chứng Stevens – Johnson/hoại tử thượng bì nhiễm độc

Hội chứng Stevens-Johnson (SJS) và hoại tử thượng bì nhiễm độc (TEN) là những phản ứng da - niêm mạc nặng với tỉ lệ tử vong cao. Thang điểm tiên lượng CRISTEN, đánh giá dựa trên 10 tiêu chí lâm sàng, và các chỉ số công thức máu có thể là giải pháp tối ưu chi phí trong việc theo dõi người bệnh. Nghiên cứu hồi cứu này được thực hiện dựa trên hồ sơ bệnh án 76 bệnh nhân SJS/TEN (32 SJS và 44 TEN) tại Bệnh viện Da liễu Trung ương và Trung tâm Dị ứng - Miễn dịch lâm sàng, Bệnh viện Bạch Mai trong 3 năm (2017 - 2019) nhằm khảo sát giá trị của thang điểm CRISTEN và công thức máu trong tiên lượng SJS/TEN. Kết quả cho thấy, nhóm TEN có số lượng bạch cầu và bạch cầu trung tính thấp hơn so với nhóm SJS (p = 0,003 và p = 0,019, tương ứng). Thang điểm SCORTEN và CRISTEN cho thấy khả năng dự đoán tình trạng nặng tốt (AUC = 0,857 và AUC = 0,815, tương ứng) và khả năng dự đoán thời gian nằm viện kéo dài khi kết hợp với chỉ số bạch cầu lympho và tiểu cầu (R2 = 0,199, p = 0,002 và R2 = 0,173, p = 0,006, tương ứng) trong phân tích hồi quy tuyến tính đa biến. Ứng dụng thang điểm SCORTEN, CRISTEN và kết quả công thức máu ngày đầu nhập viện có thể hỗ trợ đánh giá nguy cơ tình trạng nặng, thời gian nằm viện kéo dài trong SJS/TEN.
