Kết quả điều trị sau phẫu thuật ở bệnh nhân thoát vị đĩa đệm cột sống thắt lưng cùng có hẹp ống sống bằng phương pháp cố định cột sống qua cuống và ghép xương liên thân đốt qua lỗ liên hợp (xâm lấn tối thiểu và mổ mở)

Phẫu thuật xâm lấn tối thiểu TLIF (Mis-TLIF) ngày càng được sử dụng nhiều trong các bệnh thoát vị đĩa đệm cột sống thắt lưng cùng có hẹp ống sống. Mục tiêu nghiên cứu của chúng tôi nhằm đánh giá kết quả lâm sàng của phẫu thuật xâm lấn tối thiểu (Mis) và phẫu thuật mổ mở có hàn xương liên thân đốt qua lỗ liên hợp (TLIF). Phương pháp nghiên cứu mô tả, hồi cứu được thực hiện từ tháng 1 năm 2020 tới tháng 12 năm 2020 trên 83 bệnh nhân được phẫu thuật tại Bệnh viện Đại học Y Hà Nội. Độ tuổi trung bình của các bệnh nhân là 49,31 ± 12,83 tuổi, trong đó tỉ lệ Nam/Nữ là 1,01/1. So với những bệnh nhân được mổ mở TLIF, bệnh nhân được mổ theo phương pháp xâm lấn tối thiểu TLIF có lượng máu mất trong mổ, thời gian nằm viện sau mổ ít hơn hẳn (P < 0,05), thời gian phẫu thuật của nhóm xâm lấn tối thiểu TLIF cũng ngắn hơn. Hơn nữa, tình trạng đau lưng sau mổ 6 tháng ở nhóm mổ xâm lấn tối thiểu TLIF cũng ít hơn hẳn nhóm mổ mở TLIF. Tình trạng đau chân, điểm chức năng cột sống ODI sau mổ thấp hơn ở nhóm mổ xâm lấn tối thiểu TLIF tuy nhiên sự khác biệt là không có ý nghĩa thống kê. Phẫu thuật xâm lấn tối thiểu TLIF ở các bệnh nhân thoát vị đĩa đệm cột sống thắt lưng cùng có hẹp ống sống thực sự an toàn và có lợi ích nhiều hơn so với mổ mở TLIF.
