Hiệu quả can thiệp dinh dưỡng cho ung thư vòm mũi họng: Tổng quan và phân tích meta

Suy dinh dưỡng xảy ra ở 53,6 – 80,5% bệnh nhân ung thư vòm mũi họng trong hóa xạ trị, ảnh hưởng nghiêm trọng đến kết quả điều trị. Bằng chứng về hiệu quả can thiệp dinh dưỡng đặc thù cho nhóm này còn hạn chế cần có đánh giá hệ thống hiệu quả can thiệp dinh dưỡng lên kết cục điều trị và chất lượng cuộc sống ở nhóm bệnh nhân trên. Tổng quan này tiến hành theo PRISMA 2020 (PROSPERO: CRD42022366089), tìm kiếm 6 cơ sở dữ liệu. Phân tích 10 RCT (n = 879) và 4 nghiên cứu phi-RCT; đánh giá sai lệch bằng ROB2.0, NOS, NHLBI; mức bằng chứng theo GRADE. Can thiệp dinh dưỡng cải thiện có ý nghĩa prealbumin (SMD = 0,256; p = 0,032), hemoglobin, lymphocyte (SMD = 1,125; p < 0,001) và giảm 25% viêm niêm mạc miệng. Albumin và BMI cải thiện rõ ở nhóm ≥ 70 Gy. PG-SGA > 3 kết hợp NLR ≥ 3 liên quan nguy cơ tử vong tăng 4,43 lần. Can thiệp dinh dưỡng toàn diện cải thiện dinh dưỡng, giảm độc tính và nâng cao chất lượng cuộc sống bệnh nhân trên. Khuyến nghị sàng lọc PG-SGA định kỳ, đặc biệt ở nhóm liều ≥ 70 Gy.
