Kết cục lâm sàng sau 1 năm can thiệp thân chung mạch vành tại Bệnh viện Nhân dân Gia Định

Bệnh thân chung động mạch vành trái ít gặp nhưng liên quan đến tử vong; bên cạnh phẫu thuật bắc cầu, can thiệp qua da ngày càng được sử dụng rộng rãi. Hồi cứu 151 bệnh nhân can thiệp thân chung, đánh giá đặc điểm lâm sàng, thủ thuật và kết cục một năm. Tuổi trung bình 64,4 ± 11,1; hơn 80% hội chứng vành cấp, 9,9% sốc tim, trung bình GRACE 158 và phân suất tống máu 49%. Tổn thương đoạn xa 50,3%, vôi hóa 16,6%, phần lớn bệnh đa nhánh; đa số là chiến lược provisional. Thành công thủ thuật 92,1%, biến chứng 4,6%. Sau một năm, biến cố tim và mạch não nặng 7,3% và tử vong tích lũy 6,6%. Yếu tố dự báo biến cố gồm nhịp tim, Killip, nhập viện vì nhồi máu cơ tim, phân suất tống máu, GRACE, bóng đối xung động mạch chủ và kỹ thuật tối ưu đoạn gần. Kết luận: Can thiệp thân chung động mạch vành trái có tỷ lệ thành công cao, kết cục một năm thuận lợi, tình trạng lâm sàng nặng lúc nhập viện liên quan rõ với kết cục trong phân tích thăm dò.
