35. Năng lực và sự tham gia của hệ thống y tế tư nhân trong công tác chẩn đoán và sàng lọc người bệnh lao tại Hà Nội năm 2020

Nhằm đánh giá năng lực và sự tham gia của hệ thống y tế tư nhân trong công tác chẩn đoán và sàng lọc người bệnh lao, chúng tôi tiến hành nghiên cứu mô tả cắt ngang trên 100 cơ sở y tế tư nhân tại Hà Nội từ tháng 1/2020 đến tháng 12/2020. Kết quả nghiên cứu cho thấy, tỉ lệ cơ sở y tế tư nhân có cán bộ y tế được tập huấn lao là 17%. Khoảng 80% cơ sở y tế tư nhân không có nguồn tài chính cho hoạt động chăm sóc lao. Tỉ lệ cơ sở y tế tư nhân có cơ sở vật chất phục vụ chụp X-quang là 42%. Tỉ lệ này đối với AFB đờm, GenXpert, cấy đờm dao động từ 10 - 25%. Tỉ lệ chụp X-quang, soi đờm, GeneXpert, cấy đờm lần lượt là 24,3%; 2,3%; 1,2% và 0%. Tỉ lệ báo cáo ca bệnh, điều trị và chuyển viện lần lượt là 4,3%; 0,02% và 3,98%. Số lượng bác sĩ, điều dưỡng, có cán bộ y tế được tập huấn, có nguồn tài chính độc lập cho chăm sóc lao, có cơ sở vật chất phục vụ xét nghiệm vi khuẩn học là những yếu tố liên quan tới sự thay đổi số lượng báo cáo và chuyển gửi người bệnh lao. Kết quả nghiên cứu cho thấy những rào cản cần sớm được khắc phục trong chăm sóc lao tại khu vực y tế tư nhân.
