16. Đặc điểm lâm sàng và miễn dịch của bệnh Lupus ban đỏ hệ thống ở trẻ em theo tiêu chuẩn EULAR/ACR 2019

Tiêu chuẩn phân loại của EULAR/ACR 2019 là tiêu chuẩn phân loại mới cho lupus ban đỏ hệ thống với ngưỡng ≥ 10 điểm. Nghiên cứu cắt ngang trên 83 trẻ được chẩn đoán lupus ban đỏ hệ thống mới tại Trung tâm Nhi khoa, Bệnh viện Bạch Mai nhằm mô tả đặc điểm lâm sàng và miễn dịch của bệnh theo tiêu chuẩn EULAR/ACR 2019. Đặc điểm lâm sàng: tổn thương thận 63,9%, sốt 50,6%, biểu hiện da niêm mạc 49,4%, huyết học 45,8%, khớp 27,7%, thanh mạc 21,7%, tâm thần kinh 7,2%. Đặc điểm miễn dịch: kháng thể kháng nhân là tiêu chuẩn đầu vào, dương tính trong 100% trường hợp. Tỉ lệ dương tính của kháng thể kháng dsDNA, kháng thể kháng Sm và kháng thể kháng phospholipid tương ứng là 95,2%, 43,4% và 28,9%. Nồng độ bổ thể giảm trong 90,4% trường hợp. 2/3 trường hợp có tổng số điểm lâm sàng và miễn dịch ≥ 20 điểm, trung bình 24,1 ± 7,4 điểm.
