Viêm ruột lupus là một biến chứng nghiêm trọng, hiếm gặp của lupus ban đỏ hệ thống (SLE). Chẩn đoán rất khó khăn, đặc biệt khi không có các triệu chứng khác của SLE đang hoạt động. Chúng tôi báo cáo ca bệnh trẻ nữ 11 tuổi đến khám vì đau bụng, nôn nhiều, đại tiện phân lỏng 3 - 4 lần/ ngày, không có nhày máu, khám trẻ tỉnh, bụng mềm không có phản ứng thành bụng, X-quang bụng bình thường, siêu âm bụng có hình ảnh dày thành một số quai ruột, ít dịch. Sau 1 tuần, trẻ nôn nhiều hơn, nôn dịch xanh, vàng, đại tiện phân tóe nước có ít nhày không máu 12 - 15 lần/ ngày và sốt. Chụp cắt lớp ổ bụng có dày thành lan tỏa ruột non và đại tràng, nhiều dịch tự do, bạch cầu phân (+), giảm bạch cầu lympho máu, CRP bình thường, C3, C4 giảm mạnh, ANA (+), kháng thể kháng dsDNA tăng, có protein niệu. Trẻ được chẩn đoán viêm ruột/viêm thận lupus, đáp ứng tốt với corticoid sau 2 tuần. Ca bệnh muốn nhấn mạnh đến đặc điểm lâm sàng viêm ruột lupus, chẩn đoán và điều trị.
