Tình trạng nội mô giác mạc trên mắt bị rách màng descemet sau đẻ có can thiệp Forceps

Nam, 21 tuổi, đến khám để phẫu thuật cận thị hai mắt. Bệnh nhân có tiền sử can thiệp forceps khi sinh. Khám lâm sàng thấy có vết sẹo cung mày góc ngoài bên phải, giác mạc mắt phải trong, có vết rách màng Descemet chạy theo hướng chéo từ vùng rìa 6 giờ đến gần rìa 2 giờ kèm theo bong màng Descemet tại mép rách phía thái dương. Vùng bong màng Descemet chiếm 14,5% diện tích giác mạc. Mật độ tế bào nội mô trung bình bên mắt bị tổn thương là 1365 tế bào/mm2, bằng một nửa so với mắt lành. Tại vùng bong màng Descemet có sự di cư và giãn rộng của tế bào nội mô với hệ số biến thiên tế bào cao (CV 73%), tỉ lệ tế bào 6 cạnh thấp (44%). Bệnh nhân được chẩn đoán rách màng Descemet mắt phải do tiền sử lúc sinh có can thiệp forceps. Mặc dù diện tích rách và bong màng Descemet chiếm 14,5% diện tích giác mạc, giác mạc vẫn trong suốt nhờ sự giãn ra và bù trừ chức năng của các tế bào nội mô lân cận. Bệnh nhân cần được theo dõi định kì để đánh giá thay đổi tế bào nội mô và ghép nội mô giác mạc khi có tăng độ dày giác mạc do tế bào nội mô mất bù.
