Occular endothelial cell condition with descemet membrane rupture relating to previous history of Forceps delivery

Abstract

A 21-year-old myopic male was referred for refractive surgery on both eyes. A history of eye traumatic forceps-assisted delivery was noted. Eye examination revealed an external eyebrow scar on the right and an oblique tear in Descemet membrane in the right eye with Descemet’s membrane detachment at the temporal edge, which accounted for 14.5% of the total corneal area. Mean corneal endothelial cell density in the affected eye was 1365 cells/ mm², which equal to half number of the normal eye. Migration and expansion of the adjacent endothelium cells were recorded in the area with Descemet membrane detachment, demonstrated via high coefficient of variance (CV 73%) and low rate of hexagon cells (44%). Although Descemet membrane rupture accounted for 14.5% of the corneal posterior surface, the cornea was found transparent. Regular follow-up was recommended to evaluate the changes in corneal endothelial cell morphology and density. Later in life, patient might need endothelial keratoplasty when endothelial decompensation develops.
