Bệnh viêm hạch Kikuchi ở trẻ em: Báo cáo ca bệnh và tổng quan tài liệu

Bệnh Kikuchi-Fujimoto là một tình trạng hiếm gặp ở trẻ em đặc trưng bởi viêm hạch hoại tử và sốt, bệnh liên quan đến rối loạn miễn dịch, chẩn đoán xác định bệnh dựa vào xét nghiệm mô bệnh học tổ chức hạch. Chúng tôi báo cáo một ca bệnh trẻ nữ, 11 tuổi, vào viện vì sốt và sưng đau vùng cổ trái ngày thứ 12. Trẻ được điều trị ban đầu theo hướng viêm hạch nhiễm trùng nhưng đáp ứng kém với liệu pháp kháng sinh, sau đó trẻ được tầm soát một số bệnh tự miễn nhưng chưa đủ tiêu chuẩn. Cuối cùng, chúng tôi tiến hành sinh thiết hạch làm mô bệnh học để chẩn đoán. Kết quả mô bệnh học phù hợp với chẩn đoán: Bệnh Kikuchi - Fujimoto. Bệnh nhân hết sốt sau khoảng 3 tuần điều trị, được theo dõi tại nhà, đánh giá lại sau 2 tuần, tình trạng ổn định. Kết luận: Bệnh Kikuchi - Fujimoto là một tình trạng viêm hiếm gặp nhưng tự giới hạn ở trẻ em, chẩn đoán xác định dựa vào mô bệnh học. Bệnh có tỷ lệ tái phát nên cần theo dõi bệnh nhân sau khi ra viện.
