Giãn bạch mạch ruột tiên phát ở một trẻ có tiền sử u bạch huyết mạc treo đã phẫu thuật

Giãn bạch mạch ruột tiên phát (PIL) là một rối loạn giãn bạch mạch ruột bẩm sinh, hiếm gặp. Các triệu chứng lâm sàng của bệnh gây ra do mất quá nhiều dịch bạch huyết vào lòng ruột, bao gồm protein, chất béo và tế bào lympho. Chúng tôi báo cáo một trường hợp trẻ nữ, 14 tuổi, biểu hiện tiêu chảy kéo dài và phù hai chân. Trẻ có tiền sử phẫu thuật cắt u bạch huyết mạc treo ruột lúc 3 tuổi; biểu hiện tiêu chảy và phù nhiều đợt, xuất hiện từ sau phẫu thuật một năm. Xét nghiệm cho thấy giảm albumin máu (19,9 g/L), giảm nồng độ 25OH vitamin D (1,8 ng/mL) và calci máu (Ca ion 0,81 mmol/L), giảm bạch cầu lympho (0,62 G/L), protein niệu âm tính. MRI ổ bụng không có hình ảnh bất thường. Nội soi tá tràng, hồi tràng và sinh thiết có hình ảnh giãn bạch mạch ruột. Chụp bạch mạch số hóa xóa nền không có đường rò bạch mạch nhưng có giãn bạch mạch gan. Chúng tôi chẩn đoán trẻ bị PIL, điều trị bằng chế độ ăn với thành phần lipid chuỗi trung bình, sử dụng octreotide tiêm dưới da và tiêm xơ bạch mạch gan; trẻ hết phù, albumin máu duy trì ổn định.
