Giá trị của tỷ lệ thể tích lách trên tổng thể tích gan-lách [SV/(LV+SV)] trên cắt lớp vi tính trong đánh giá xơ gan

Nghiên cứu nhằm khảo sát mối liên quan và giá trị thăm dò của tỷ lệ thể tích lách trên tổng thể tích gan-lách [SV/(LV+SV)] trên cắt lớp vi tính (CLVT) trong đánh giá xơ gan theo Child-Pugh. Nghiên cứu cắt ngang hồi cứu có nhóm bệnh gồm 123 bệnh nhân xơ gan có dữ liệu Child-Pugh và nhóm chứng gồm 50 đối tượng không xơ gan. Thể tích gan, lách được đo trên thì tĩnh mạch cửa; tỷ lệ SV/(LV+SV) được tính theo phần trăm. Nhóm xơ gan có thể tích gan thấp hơn, thể tích lách và tỷ lệ SV/(LV+SV) cao hơn nhóm chứng (p ≤ 0,005). Trong nhóm xơ gan, thể tích gan có xu hướng giảm, còn thể tích lách và tỷ lệ SV/(LV+SV) tăng dần từ Child-Pugh A đến C (p < 0,001). Tỷ lệ SV/(LV+SV) tương quan thuận mức trung bình với Child-Pugh (Spearman rho = 0,554; p < 0,001). AUC (khoảng tin cậy 95%) khi phân biệt A-B, B-C và A-C lần lượt là 0,666 (0,550 - 0,782), 0,781 (0,679 - 0,883) và 0,869 (0,788 - 0,950). Tỷ lệ SV/(LV+SV) có thể cung cấp thông tin CLVT bổ trợ, nhưng các điểm cắt cần được kiểm định thêm.
