Abstract

This retrospective cross-sectional study explored the association and preliminary discriminative value of the spleen-to-total liver-spleen volume ratio [SV/(LV+SV)] measured on computed tomography (CT) in Child-Pugh-based cirrhosis assessment; This study included a control group of 123 cirrhotic patients with available Child-Pugh data and 50 non-cirrhotic controls. Liver and spleen volumes were measured on portal venous phase CT and SV/(LV+SV) was calculated as a percentage. The cirrhotic group had lower liver volume and higher spleen volume and SV/(LV+SV) than controls (p ≤ 0.005). Across Child-Pugh classes, liver volume decreased, whereas spleen volume and SV/(LV+SV) increased from A to C (p < 0.001). SV/(LV+SV) showed a moderate positive correlation with Child-Pugh severity (Spearman's rho = 0.554; p < 0.001). The AUCs (95% confidence intervals) for differentiating A-B, B-C, and A-C were 0.666 (0.550 - 0.782), 0.781 (0.679 - 0.883), and 0.869 (0.788 - 0.950), respectively. SV/(LV+SV) may provide complementary CT-based information, but the proposed cut-offs require further validation.
