Đặc điểm và diễn tiến sớm tổn thương động mạch vành không thủ phạm trên Ivus ở bệnh nhân STEMI

Nghiên cứu nhằm đánh giá đặc điểm hình thái và diễn tiến sớm của các tổn thương động mạch vành không thủ phạm bằng siêu âm trong lòng mạch (IVUS) ở bệnh nhân nhồi máu cơ tim cấp có ST chênh lên (STEMI). Nghiên cứu tiến cứu trên 103 bệnh nhân STEMI với 118 tổn thương động mạch vành không thủ phạm được đánh giá bằng IVUS tại thời điểm ban đầu; trong đó 32 tổn thương được khảo sát IVUS lần 2. Mảng xơ vữa hỗn hợp là hình thái thường gặp nhất (41,5%). Các đặc điểm nguy cơ cao gồm MLA < 4 mm2 (68,6%), gánh nặng mảng xơ vữa ≥ 70% (52,5%) và vôi hóa điểm (52,5%). Tổn thương có từ hai đặc điểm nguy cơ cao trở lên chiếm 66,9%. Ở nhóm lặp lại IVUS, 65,6% tổn thương tiếp tục tồn tại ≥ 2 đặc điểm nguy cơ cao. Như vậy, ở bệnh nhân STEMI nhiều nhánh, tổn thương không thủ phạm được khảo sát bằng IVUS thường có nhiều đặc điểm nguy cơ cao; trong nhóm được IVUS lặp lại sau thời gian theo dõi ngắn, đa số các đặc điểm này vẫn tồn tại
