Characteristics and Early Evolution of Non-Culprit Coronary Lesions on Intravascular Ultrasound in STEMI Patients

Abstract

This study aimed to evaluate the morphological characteristics and early evolution of non-culprit coronary lesions using intravascular ultrasound (IVUS) in patients with ST-segment elevation myocardial infarction (STEMI). A prospective study was conducted in 103 STEMI patients, including 118 non-culprit coronary lesions assessed by IVUS at baseline; among these, 32 lesions underwent repeat IVUS evaluation. Mixed plaques were the most common plaque phenotype (41.5%). High-risk plaque features included a minimum lumen area (MLA) <4 mm2 (68.6%), plaque burden ≥ 70% (52.5%), and spotty calcification (52.5%). Lesions presenting with two or more high-risk features accounted for 66.9%. In lesions with repeat IVUS assessment, 65.6% continued to exhibit at least two high-risk features. Thus, in patients with multivessel STEMI, non-culprit coronary lesions assessed by IVUS frequently exhibited multiple high-risk plaque features. Among lesions undergoing repeat IVUS after a short follow-up period, most of these features remained present.
