Thang điểm milan trên đo áp lực nhu động thực quản độ phân giải cao ở bệnh nhân có triệu chứng trào ngược

Nghiên cứu cắt ngang trên 141 đối tượng ≥ 18 tuổi có triệu chứng trào ngược với điểm GerdQ ≥ 8 được đo áp lực nhu động thực quản độ phân giải cao (HRM) và nội soi dạ dày - thực quản tại Viện Nghiên cứu và Đào tạo Tiêu hoá, Gan mật và Phòng khám Đa khoa Hoàng Long từ 10/12/2025 đến 31/03/2026, nhằm khảo sát thang điểm Milan và một số yếu tố liên quan ở bệnh nhân có triệu chứng trào ngược. Điểm Milan trung vị của nhóm nghiên cứu là 85, nhóm có điểm Milan ≥ 137, tương ứng nguy cơ cao mắc trào ngược dạ dày thực quản (GERD) chiếm 37,6%. Trong phân tích hồi quy logistic đa biến, BMI, triệu chứng nóng rát, thời gian xuất hiện triệu chứng và viêm thực quản trào ngược (VTQTN) theo phân loại Los Angeles là các yếu tố liên quan độc lập có ý nghĩa thống kê với nguy cơ cao mắc GERD theo thang điểm Milan. Cụ thể, BMI (OR = 1,192; KTC 95%: 1,008 - 1,411; p = 0,041), triệu chứng nóng rát (OR = 3,255; KTC 95%: 1,372 - 7,720; p = 0,007), thời gian triệu chứng (OR = 1,026; KTC 95%: 1,003 - 1,050; p = 0,029) và có VTQTN trên nội soi (OR 3,271; KTC 95%: 1,378 - 7,766; p = 0,007). Thang điểm Milan có liên quan với đặc điểm lâm sàng và tổn thương niêm mạc thực quản, qua đó có thể hỗ trợ nhận diện nhóm bệnh nhân có nguy cơ cao mắc trào ngược.
