Kháng thể bất thường (KTBT) làm giảm hiệu quả truyền máu ở bệnh nhi thalassemia. Nghiên cứu cắt ngang được thực hiện trên 253 bệnh nhi alpha thalassemia (α- thalassemia) có hồ sơ truyền máu và kết quả xét nghiệm KTBT đầy đủ, sử dụng kỹ thuật ngưng kết cột gel với panel hồng cầu của Viện Huyết học - Truyền máu Trung ương và trên máy định nhóm máu tự động của hãng Grifols. Phân tích thống kê bằng phần mềm SPSS 27.0. Kết quả cho thấy tỷ lệ KTBT là 8,7% (22/253 trường hợp), trong đó hệ Rh chiếm tỷ lệ cao nhất (63,34%), tiếp đến là hệ MNS (30,0%); anti-E và anti-Miᵃ là các kháng thể thường gặp nhất. Tỷ lệ KTBT cao hơn ở trẻ dân tộc Kinh so với nhóm dân tộc khác (12,7% so với 5,2%; p = 0,044) và tỷ lệ xuất hiện KTBT tăng rõ rệt theo số lượng khối hồng cầu đã truyền (p < 0,0001).
