The result of deep anterior lamellarkeratoplasty for descemetocele keratitis

Abstract

This is a prospective study aimed to evaluate the use of deep anterior lamellar keratoplasty to treat infectious corneal descemetocele with. This study analyzed data from 24 patients (24 eyes) who had deep anterior lamellar keratoplasty for infectious corneal descemetocele at the Vietnam National Institute of Ophthalmology between January 2018 and June 2020. The patients were 50% male and 50% female. The mean age was 58.3 ± 13.9. The causes of infectious corneal descemetocele were bacteria (29.2%), Herpes Simplex (62.4%), fungi (4.2%) and Microsporidia (4.2%). Eyeball integrity was preserved in 22/24 eyes (91.7%). Infectious recurrence was identified in 2 eyes (8.3%). The mean postoperative visual acuity (LogMAR) was 1.63 ± 0.28 and was higher than the mean preoperative visual acuity (LogMAR) 2.09 ± 0.16 (P < 0,05). There were 45.8% eyes with graft clarity and 8.4% with hazy graft at the last follow-up. Surgical complications included Descemet rupture (20.8%), double anterior chamber (41.7%), Descemet folds (58.3%) and interface vascularization (41.7). In conclusion, deep anterior lamellar keratoplasty is likely to be effective and safe in the treatment of infectious corneal descemetocele, particularly, in situation where emergency penetrating keratoplasty is not feasible.
