30. The prognostic value of blood lactate levels in patients undergoing valvular surgery using cardiopulmonary bypass

Abstract

The objective of the study was to evaluate the prognostic value of blood lactate levels in patients undergoing valvular surgery using cardiopulmonary bypass. The study was conducted on 348 patients with an average age of 54.9 ± 11.7; mean of Euroscore II (%) was 4.07 ± 6.27. The results showed that the lactate level at 6 hours post-operation (T6h) was valuable in predicting the early postoperative mortality (AUC = 0.883, with a cut-off point of 4.0 mmol/L, sensitivity was 77, 8% and specificity of 78.7%) and predictive of major postoperative complication (AUC = 0.910, with a cut-off of 4.0 mmol/L, sensitivity of 76.1% and specificity of 85.7%). T6h lactate levels ≥ 4 mmol/L and Euroscore II (%) were independent predictors of major postoperative complications with OR of 14.650 (p = 0.001) and 1.014 (p = 0.001), respectively and an independent predictor of risk of 30 days postoperative mortality with HR of 6.097 (p = 0,041) and 1.072 (p = 0.047), respectively.
