31. Effectiveness of community health workers in promoting hypertension adherence among elderly patients in district 10 in Ho Chi Minh City

Abstract
This is an intervention study to evaluate the improvement of treatment adherence in hypertensive patients after applying the community health workers model in monitoring, disease management, and health education for 6 months. The study period was from November 2020 to May 2021, with ward 10 as the intervention group and ward 6 as the control group. After the intervention, the rate of drug adherence increased in both groups, with no significant difference between them (PR = 2.06 and 1.97; p > 0.05). The rate of patients who had regular follow-up visits decreased in both groups, where the intervention group decreased significantly less than the control group (PR = 0.86 and 0.69; p < 0.001). The proportion of patients with controlled hypertension increased by 7% in the intervention group, while the control group decreased by 27% (p < 0.001). The study has demonstrated the initial impact of the community health workers model in supporting treatment adherence and improving hypertensive patients’ disease status.
