39. Bronchiolitis obliterans as a long-term sequela of mycoplasma pneumoniae-induced-stevens johnson: A case report

Abstract

Steven-Johnson syndrome (SJS) is an immune-mediated hypersensitivity reaction, mainly related to medications and infections. It is characterized by blisters and extensive necrosis of the skin and mucous membranes, acute and usually self-limiting. Bronchiolitis Obliterans (BO) is a rare respiratory complication of SJS which occurs after the acute phase. We report a case of a 32-month-old boy diagnosed with SJS due to Mycoplasma pneumoniae, presenting with fever, cough, blister, oral mucosal ulcer and conjunctivitis, positive Mycoplasma pneumoniae test (IgM > 1352 U/mL). Skin and mucosal  lesions improved after 2 weeks, but after 5 weeks, he  developped wheezing and dyspnea. HRCT showed mosaic pattern, bronchial wall thichkening and air trapping. BO is a late respiratory complication of SJS. Long-term monitoring of respiratory manifestations in SJS patients is essential to have prompt interventions, avoiding more serious lung damage.
