16. Value of dual-energy computed tomography in detecting Bone marrow edema in vertebral compression fractures

Abstract
This study aimed to evaluated the value of dual-energy computed tomography (DECT) in the detection of bone marrow edema in patients with vetebral compression fractures. 18 patients with 128 vetebral bodies undergoing both DECT and MRI were evaluated. The results show that the color-coded VNCa images for all vertebrae in the diagnosis of bone marrow edema have sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of 84.2%, 100%, 97.3% and 97.7%, respectively. Also, the difference in density between vertebrae with and without bone marrow edema was statistically significant (p < 0.0001). With an overall AUC of 0.935 and cutoff value of -13.1HU, the sensitivity and specificity of DECT as compared to MRI in detecting bone marrow edema were 89.5% and 91%. As such, DECT is a useful diagnostic tool compared to conventional CT, which can provide addtional information on recent vetebral fractures and bone marrow edema. DECT can also be used as an adjunct to or as an alternative to MRI in patients with contraindications.
