17. Outcomes of surgical management for abdominal lymphangioma in pediatric patients at the National Children Hospital

Abstract

Abdominal lymphangioma is a malformation of the lymphatic system that is present with cystic dilated lymphatics in the abdominal capacity. A retrospective review study was conducted on 83 patients (51 males, 32 females) who underwent surgical resection of abdominal lymphangioma at the National Children Hospital between 2017 and 2021. Result: The mean age of these patients was 45.63 months. The common clinical presentation was abdominal pain (49.4%), a palpable mass (26.5%) and abdominal extension (22.9%). The cysts were intra-operative found in the bowel mesentery, omentum, and retroperitoneal space with the rates were 43.3%; 34.9% and 21.8%, respectively. Total cyst excision was performed in 38 cases (45.8%), simultaneous bowel resection was needed in 26 mesenteric cysts (31.3%), partial cystectomy and sclerosis therapy with Bleomycin was applied in 19 cases (22.9%). There were no perioperative complications in either method of treatment. Subsequently, 13 cases developed recurrence cysts (15.7%); of which, 9 cases had a spontaneous regression without treatment, while 4 cases needed a second intervention included cyst resection and re-do sclerosis therapy. In the retroperitoneal group, the rate of recurrence was 55.5% (10/18 cases). Of the 19 cases who underwent intracystic injection with Bleomycin, recurrence was identified in 8 cases (42.1%). The mean follow-up was 19.5 months, no more recurrence of the cystic lymphangioma was noticed. From the results, surgical management of the abdominal lymphangioma at National Children Hospital effective, safe, with low rate of recurrence.
