25. Early outcomes of laparoscopic total colectomy by duhamel technique at the national children's hospital in the period 2013 - 2022

Abstract

A descriptive study was conducted to evaluate the early outcomes of laparoscopic total colectomy by Duhamel technique at the National Children's Hospital from 2013 to 2022. The study included 52 pediatric patients with total colonic aganglionosis. Clinical, paraclinical information, surgical characteristics and early postoperative outcomes were collected. The study results showed that the average surgical time was 158.3 ± 39.9 (minutes), the average length of the ileum resection was 18.1 ± 10.2 (cm). No complication or death was recorded. The mean time of intestinal transit was 8.7±4.0 (hours), the mean time to oral feeding after surgery was 1.2 ± 0.4 days, The mean postoperative hospital stay was 8.4 ± 3.5 days. Only 2 patients (3.8%) had wound infection, 1 patient (1.9%) had ileal perforation. After surgery, 46 patients (88.5%) had good bowel function, 5 patients (9.6%) had moderate bowel function and 1 patient (1.9%) had poor bowel function. The study showed that laparoscopic surgery for children with total colonic aganglionosis is safe, highly feasible, has a fast recovery time, a short hospital stay, and the child's postoperative defecation function is at a good level.
