17. Outcomes of nutritional intervention in children with chylothorax after cardiac surgery in Vietnam National Children’s Hospital

Abstract

Nutritional treatment has demonstrated beneficial effects in managing chylothorax in children after heart surgery. A cross-sectional study with 26 children treated for chylothorax after cardiac surgery was conducted at the National Children’s Hospital from 01/01/2022 to 30/08/2023. The cases comprised of 61.5% male and 38.5% female, with ages ranging from 13 days to 45 months; the majority were within 1 - 12 months old (65.4%). The most common causes of chylothorax was Tetra of Fallot and Coarctation of the aorta, accounting for 19.2%. There were 21 children underwent MCT-rich diet treatment (MCT-diet), while 5 children received total parenteral nutrition method (TPN). Following treatment, 25 showed successful recovery, with the remaining child needing invasive intervention. Weight loss rate of 42.3% was observed in the study. Treatment-related complications included sepsis (34.6%), hypoalbuminemia (53.8%), elevated liver enzymes (11.5%), and renal abnormalities (61.5%). Median drainage time, mechanical ventilation time, and length of hospital stay were 6 days, 4 days, and 29 days, respectively. Nutritional treatment can be an effective treatment option for chylothorax in children.
