42. “Lollipop” sign of hepatic epithelioid hemangioendothelioma: A case series

Abstract

Epithelioid hemangioendothelioma (HEHE) is a rare hepatic neoplasm that poses significant diagnostic challenges due to its imaging similarities with other liver tumors. This report aims to elucidate the distinctive radiological features of HEHE using three illustrative cases. On MRI or CT with contrast enhancement during the venous phase, HEHE typically presents as multifocal liver lesions exhibiting peripheral enhancement. A notable diagnostic indicator is the “lollipop” sign, characterized by a non-enhancing tumor mass (representing the candy) and an adjacent truncated, enhancing vein (representing the stick). This sign is crucial for radiologists to identify when evaluating potential HEHE. This imaging feature can also aid pathologists in the histopathological confirmation of HEHE.
