19. Lymph node metastasis status and immunohistochemistry characteristics of invasive micropapillary breast cancer patients undergoing surgery at K Hospital

Abstract
The study evaluates the status of axillary lymph node metastasis and immunohistochemical characteristics of 62 patients with invasive micropapillary breast cancer who underwent surgery at K Hospital from January 2018 to December 2023. The average age was 50.1 years. Most patients had BIRADS 4C or BIRADS 5 on breast ultrasound (37.1% and 46.8%, respectively). The rate of axillary lymph node metastasis was 72.6%, with stages N1, N2, and N3 being 40.3%, 22.6%, and 9.7%, respectively. The rates of ER-positive, PR-positive, and HER2/neu (+++) patients were 87.1%, 74.2%, and 29%, respectively. Patients with lymphovascular invasion accounted for a high proportion at 56.5%. Factors related to axillary lymph node metastasis included tumor size, lymphovascular invasion, and the micropapillary component of the tumor. However, there was no difference in the rate of axillary lymph node metastasis among pathologic subtypes. Invasive micropapillary breast cancer exhibits characteristics of high malignancy, with early axillary lymph node metastasis and a high rate of positive hormone receptor and low rate of HER2/neu overexpression.
