20. Primary evaluation of the results of total glosso - laryngectomy for advanced cancer of the base of tongue

Abstract

Carcinoma of the base of the tongue is typically diagnosed at advanced stage. Chemoradiotherapy aims to preserve the organ and yields favorable results; however, surgery remains a crucial treatment modality for advanced tumors and is the only recourse for patients who residue or recurrence after chemoradiotherapy. This study involved 26 patients with advanced-stage carcinoma of the base of the tongue who underwent total glosso-laryngectomy: 12 patients relapsed after chemoradiotherapy, 2 patients had carcinoma of the minor salivary glands at the base of the tongue, and 12 patients underwent primary surgery. Among these, 10 patients also underwent segmental mandibulectomy, and 3 patients underwent marginal mandibulectomy. All defects were reconstructed using a pectoralis major myocutaneous flap. Postoperative complications occurred in 23.1% of patients, with 7.7% developing fistulas. The rate of restoration to natural oral alimentation postoperatively was 65%.
