Endoscopic features and computed tomography imaging of congenital middle ear Cholesteatoma in children

Abstract

This study was to report on endoscopic features and computed tomography imaging of congenital middle ear cholesteatoma in children. A case series study was conducted on sixty-three children diagnosed with congenital middle ear cholesteatoma and underwent surgery at the National ENT Hospital, Bach Mai Hospital, and Thanh Hoa Pediatric Hospital from January 2020 to June 2024. The mean age was 4.5 ± 2.7 years old; 81% of patients were from six to under six years old. Male to female ratio was 2.7 to 1. 88,9% of the cases were incidentally detected during routine ENT examination. Otoendoscopy revealed a white mass in the mesotympanum unilaterally in all patients: 55.6% at the anterosuperior angle, 33.3% at the posterosuperior angle, 11.1% of the mass expanded to equal or more than two angles. Stages I, II, III, and IV were rated at 61.9%, 9.5%, 22.2%, and 6.3%, respectively. The overall correct diagnosis of stages using computed tomography imaging was 90%, ossicular chain destruction was 66.7%. Congenital middle ear cholesteatoma was often asymptomatic. Early diagnosis can be achieved by otoendoscopy, and computed tomography imaging was needed for staging, evaluating ossicular chain destruction, and planning appropriate surgical treatment.
