1. Serum substance P concentrations in chronic spontaneous urticaria

Abstract

A cross-sectional study was conducted on 120 patients with chronic spontaneous urticaria (CSU) and 30 healthy controls to determine the association between serum substance P levels and disease activity and response to second-generation H1 antihistamines (sgAH1) treatment. The results showed that serum substance P levels in the severe CSU group (129.54±89.85pg/mL) were significantly higher than those in the non-severe group (101.27±88.48pg/mL) (P=0.014) but not significantly different from those in the healthy control group (101.44 [70.92-170.1]pg/mL) (P=0.469). Serum substance P levels were correlated with disease activity (Spearman correlation coefficient was 0.224, P=0.014) and sex (P=0.034) but not with age, angioedema, eosinophilia, increased IgE, increased CRP, increased anti-TPO IgG (P>0.05). Univariate and multivariate logistic regression showed that the threshold value of this substance (>97.66pg/mL) was the only significant risk factor for severe CSU (OR=2,6; P<0,05). There was no difference in baseline serum substance P levels with response to sgAH1 treatment in the severe CSU group (P>0.05). In conclusion, serum substance P levels serve as biomarker predicting CSU severity but are not a predictor of response to sgAH1 in the severe group.
