Abstract

In this study, 200 Klebsiella pneumoniae strains were isolated from lower respiratory and intra-abdominal infections at Bach Mai Hospital between 2022 and 2023. Antibiotic susceptibility testing was performed using the BD Phoenix M50 system. Additionally, the presence of five common carbapenemase-encoding genes (blaNDN, blaOXA-23, blaOXA-48, blaIMP, blaKPC) was investigated using PCR. The results showed that K. pneumoniae strains from the lower respiratory tract exhibited low sensitivity to the tested antibiotics (16 - 51%), whereas those isolated from intra-abdominal infections showed higher sensitivity (51 - 71%). Carbapenemase genes were present in 42.5% of the K. pneumoniae strains, with blaKPC (21%) and blaNDM (15%) being the most prevalent genotypes; no strain carrying blaIMP was detected. A total of 51 out of 85 isolates carried multiple carbapenemase genes (e.g., blaKPC + blaOXA-48, blaKPC + blaNDM, blaKPC + blaOXA-48 + blaNDM) and exhibited high resistance to most antibiotics. The findings provide valuable data to support clinicians in selecting empirical antibiotic therapies for K. pneumoniae infections, particularly when antibiotic susceptibility testing is unavailable.
