22. Surgical outcome of the preoperative embolization meningioma at Viet Duc Hospital

Abstract
Meningiomas account for 36% of all brain tumors. Some tumors are large, hypervascular, bone invasion, and are closely related to neural structures. Total tumor removal is a major challenge. The objective of the study is to evaluate the results of meningioma surgery with preoperative embolization. A cross-sectional descriptive study was conducted on 52 patients who underwent preoperative embolization and surgery from January 2022 to October 2024 at Viet Duc Hospital. Results: The mean age was 52.23 years old and the female/male ratio was 1.74/1. Meningioma grade I lesions accounted for 76.92% and grade II accounted for 23.07%. The most common symptoms were headache (59.62%). Most tumors were located at the skull base (75%). Tumors larger than 50cm3 accounted for 69.23%. Complete occlusion of tumor-feeding vessels accounted for (84.62%), with no complication after embolization. The mean operative time was 5.48 hours. The average amount of blood transfusion was 298 ml. Total tumor removal accounted for 48.08%. The complication rate was less than 5%. Embolization before surgery for large-sized meningiomas, rich in blood vessels feeding the tumor, tumors in the skull base is a safe and effective method.
