21. Result of reconstruction after resection of basal cell carcinoma in lower eyelid

Abstract
This study was conducted to evaluate the reconstruction outcomes following the excision of lower eyelid basal cell carcinoma. This descriptive study involved 40 patients who underwent lower eyelid defect reconstruction after basal cell carcinoma excision at K Hospital from March 2018 to June 2024. Outcomes were assessed at discharge and 6 months postoperatively. A total of 40 defects (ranging in size from 0.3 to 20cm², including 18 superficial and 22 full-thickness eyelid defects) underwent superficial laminar reconstruction using the following techniques: 1 direct closure, 22 cheek rotational flaps, 8 cheek transposition flaps, 5 V-Y advancement cheek flaps, and 4 nasolabial rotation flaps. Deep laminar reconstruction involved 16 conjunctival advancement flaps and 6 cases utilizing auricular cartilage and oral mucosa grafts. At discharge, 100% of flaps/grafts demonstrated complete survival, with no reported case of bleeding or infection. At the 6-month follow-up (26 patients), 10 cases of ectropion and 14 cases of epiphora were observed, with no evidence of tumor recurrence. The results indicate that the majority of lower eyelid defects following basal cell carcinoma excision can be safely reconstructed using local flaps. However, functional and aesthetic outcomes of the lower eyelid vary depending on several factors, including the location, size, and depth of the defect, as well as the quality of the reconstructive materials.
