37. Study of clinical characteristics, risk factors, and shoulder ultrasonographic findings in patients with adhesive capsulitis at Hai Phong International General Hospital

Abstract
This cross sectional descriptive design study encompassed 118 patients diagnosed for adhesive capsulitis according to the integrated clinical and paraclinical criteria. Each participant underwent assessment of demographic variables, pain intensity on the Visual Analogue Scale (VAS), shoulder range of motion (ROM), the Shoulder Pain and Disability Index (SPADI), prevalent risk factors, and shoulder ultrasonography. The mean age was 59.5 ± 8.5 years old, with a female predominance (male to female ratio = 1 : 1.4). The interval from symptom onset to hospital admission averaged 5.4 ± 1.7 months. The mean VAS score was 6.8 ± 2.1, and all patients exhibited severe ROM limitation: flexion 72.6 ± 15.3°, abduction 65.7 ± 11.6°, and external rotation 20.5 ± 8.9°. SPADI scores were largely in the moderate to severe range. Risk factors included diabetes mellitus, hyperlipidemia, and shoulder immobilization exceeding one month. Ultrasonography demonstrated a coracohumeral ligament thickness > 3mm in approximately 80 % of patients, anterior capsule thickness > 2.5mm in 70%, and posterior capsule thickness > 2.5mm in 45%. Biceps tendon sheath synovitis was present in 35%, subacromial-subdeltoid effusion in 40%, and mild rotator cuff lesions in 25%. Thickness of the coracohumeral ligament and joint capsule correlated inversely with shoulder motion arc (p < 0.05). Integrating clinical examination with ultrasonography enables early diagnosis and quantification of shoulder stiffness severity.
