Pregnancy outcomes in patients with eisenmenger syndrome: A case series report and literature review

Abstract
This case series study presents three patients with Eisenmenger syndrome (ES) who were managed during pregnancy at Hanoi Medical University Hospital from April 2024 to March 2025. The study evaluates maternal-fetal outcomes and reviews treatment strategies for pregnant patients with ES. All three cases involved women with ES secondary to ventricular septal defects, with a mean age of 33 years old. The management of these pregnancies utilized a multidisciplinary approach, collaborating among cardiology, obstetrics, neonatology, and anesthesiology teams. Key interventions included pulmonary-targeted therapies such as sildenafil, tadalafil, and iloprost, along with close monitoring and planned cesarean deliveries at 32 - 35 weeks' gestation. All three mothers survived, although one experienced postpartum pneumonia. The newborns all achieved Apgar scores ranging from 8 to 9. This study highlights the importance of multidisciplinary care and proactive delivery planning as vital factors in achieving successful outcomes.
