39. Magnetic resonance imaging features and prognostic factors in retinoblastoma treated with intra-arterial chemotherapy: A cohort study

Abstract
This study describes the imaging characteristics of retinoblastoma (RB) primarily on magnetic resonance imaging (MRI) in comparison with fundus examination, and identifies prognostic factors for ocular salvage in patients treated with multimodal conservative therapy including intra-arterial chemotherapy. A combined retrospective-prospective cohort study was conducted on 25 eyes from 19 patients (35 tumors) at Bach Mai Hospital and the Vietnam National Institue of Ophthalmology from January 2023 to December 2024. On MRI, RB typically shows high signal intensity on T1-weighted images, low signal on T2-weighted images, high signal on FLAIR, diffusion restriction on DWI/ADC, and heterogeneous contrast enhancement. No case of optic nerve, scleral, or intracranial invasion was observed. The estimated mean ocular salvage time using the Kaplan-Meier method was 64 months, with a 5-years eye preservation rate exceeding 70%. Poor prognosis was associated with advanced IIRC groups (D, E) and the presence of intraocular hemorrhage. MRI is valuable in the diagnosis and staging of RB and complements fundoscopic examination effectively. Larger studies are needed to further validate prognostic factors.
