12. Evaluation of miRNA-29a-5p expression and its association with clinical and paraclinical characteristics in hbv-related hepatocellular carcinoma, liver cirrhosis and chronic hepatitis patients

Abstract
This study aimed to evaluate the diagnostic value of miRNA-29a-5p in hepatocellular carcinoma (HCC), particularly in early-stage disease and in the context of chronic liver disease caused by HBV infection. A cross-sectional descriptive study was conducted, quantifying serum miRNA-29a-5p levels using qPCR in two phases: screening (30 patients) and validation (300 patients). The results showed that miRNA-29a-5p was significantly overexpressed in the HCC group compared to the hepatitis B and cirrhosis groups, with corresponding fold-changes of 2.69 and 2.07 (p < 0.001). The area under the curve (AUC) of miRNA-29a-5p for distinguishing HCC from chronic hepatitis B was 0.677 (p < 0.001), and for distinguishing HCC from cirrhosis was 0.758, which was higher than AFP (AUC = 0.729). In patients with HCC, when comparing the expression levels of miRNA-29a-5p between the two groups with AFP ≤ 20 ng/mL and > 20 ng/mL, the difference was statistically significant (p = 0.032).
