71. Current status of blood gas external quality assessment participation among laboratories in Northern Vietnam, 2020-2024

Abstract

Objective: To evaluate the current status of participation and performance in blood gas (BG) external quality assessment (EQA) among medical laboratories in Northern Vietnam during 2020 – 2024. Materials and Methods: A cross-sectional descriptive study analyzing BG EQA data coordinated by the Center for Standardization and Quality Control in Medical Laboratories – Hanoi Medical University. Laboratories were classified by hospital tier and geographical region. The program consisted of 12 rounds per year, each with one liquid EQA sample including 11 parameters (pH, pCO2, pO2, HCO3, tCO2, glucose, lactate, Na+, K+, Cl, Ca2+). Results were evaluated using the Standard Deviation Index (SDI) and Target Score (TS), with satisfactory performance defined as |SDI| ≤ 2 or TS ≥ 50. Results: The number of participating laboratories increased from 18 in 2020 to 39 in 2024 (77% increase). The proportion of laboratories achieving ≥ 90% satisfactory results rose from 50.0% in 2020 to 72.0% in 2024. pH and lactate consistently achieved > 90% satisfactory rates, while pO2, pCO2, and glucose remained between 80–90%. Participation was concentrated in the Red River Delta and Northeastern regions, while other areas were less represented. Conclusion: Blood gas EQA participation in Northern Vietnam has improved both in scale and performance, though disparities remain across parameters and regions. Strengthening training, supervision, and expanding participation are needed to enhance blood gas testing quality and move toward international standardization.
