Failure of NSAID Therapy in Primary Dysmenorrhea: A Cohort Study at Hung Vuong Hospital

Abstract
This study aimed to estimate the short-term failure rate of non-steroidal anti-inflammatory drugs (Ibuprofen 400mg) in treating primary dysmenorrhea, and identify associated factors. A prospective cohort study involving 206 women of reproductive age with primary dysmenorrhea was conducted at Hung Vuong Hospital from December 2024 to April 2025. Pain was assessed using the Visual Analog Scale (VAS) before treatment, after 3 days, and after 3 menstrual cycles. Treatment failure was defined as a < 2-point reduction in VAS after 3 days. The treatment failure rate after 3 days and 3 months of NSAID use was 16.02%, and 2.98%, respectively. Factors associated with treatment failure included: women with normal BMI had a lower risk of failure compared to underweight women (RR = 0.48; p = 0.014); greater menstrual blood loss was associated with a higher risk of treatment failure (RR = 1.11; p < 0.001); and treatment adherence was associated with a significantly lower risk of failure (RR = 0.41; p < 0.001). NSAIDs remain a feasible and effective first-line option in the clinical management of primary dysmenorrhea. Certain factors associated with treatment failure should be identified early to individualize therapy, minimize symptoms, and improve patients’ quality of life.
