Sutureless aortic valve replacement in bicuspid aortic valve at Bach Mai Hospital

Abstract

This study reports the initial experience with sutureless aortic valve replacement in patients with bicuspid aortic valve. Three patients with bicuspid aortic valve underwent aortic valve replacement with the Perceval Plus sutureless prosthesis at Bach Mai Hospital. All patients had type 1 (Sievers). Traditional full sternotomy was used in all cases, followed by excision of the diseased valve, annular decalcification, sizing, and valve deployment according to standard procedure. The mean cardiopulmonary bypass and aortic cross-clamp times were 88.7 ± 18.1 and 64.0 ± 21.9 minutes, respectively. Postoperative recovery was uneventful. At discharge, echocardiography demonstrated significant improvement in effective orifice area and low transvalvular gradients. Sutureless aortic valve replacement in bicuspid aortic valve appears to be a feasible option, reducing operative times while ensuring favorable hemodynamic performance and patient safety.
